Osaka, Kobe, Nagasaki would likely have the most/most-accessible options but there should be possibilities for each location.
I have not personally been on a cruise, but have heard that the biggest challenges for accessibility tend to be getting out of the port itself (ie the buses provided only have stairs). However, if the wheelchair can fold and getting up the steps into the coach bus is possible, then you can likely join many of the excursions.
