I think you have some information already on tours. As far as hotels, it depends on where in the city you’d like to stay. I can recommend the Tokyo Station Maronuchi and Hotel Celestine Ginza. I can get along without a fully disabled room and these had separate showers (not bathtub only- which is a deal breaker for me).
It’s not always fool proof but you can check website pictures to see what the accessibility in the rooms look like.
