I recently did the form and I got told to contact Ohayo Travel. They unfortunately only do pick up and returns in Tokyo but they have both manual and electric wheelchairs and the delivery is included in the price. How much it is depends on how long you rent it but up to 7 days is 29,000 yen for the standard manual, 32,000 for the large manual, and 46,000 for the electric. Up to 14 days is 37,000, 40,000, and 53,000 respectively. It won’t let me upload the full pdf but I’ve attached a screenshot with the details of the electric wheelchair, hope that helps!
