Finding a really ‘traditional’ ryokan may be tough, depending on your definition. A quick google search brings up the following for a place to stay: http://www.pax-yoshino.com/
It looks like they have some barrier-free rooms.
As for how to get to Hakone from Kyoto, it looks like there’s a fair number of transfers involved. It really depends on what station you’re leaving from. If you give me some more information, I can try and help you out!
