Didn’t find anything in Tunisia (though might be good to try in French) but when asking around, someone suggested London Cab Egypt in Egypt:

They said the design can accept a wheelchair user and the FAQ backs this up, but it may just be referring to a hospital like wheelchair. I’d suggest getting in touch and telling them about your wheelchair size/weight before booking.
Also, the person who told me about them said it was a bit expensive to use for touring since it is a taxi company…
