I submitted my request for special assistance and my booking says the request is registered but I always like to confirm things by phone especially as it gets closer to flight date but it has been impossible to connect by phone. First of all onsen will be difficult. In general they are always separated between men and women in the main baths so your only option would be an onsen that has what would be called a private bath or sometimes a family bath.
